RELEASE OF LIABILITY:

Yoga (hereafter, "Classes,") can involve physical exertion as well as relaxation and stress-reduction. It is my sole responsibility to ensure that I am physically qualified to take Classes Hollywood Forever (hereafter, "HF.") As with any physical activity, there is the possibility of injury, or even death. I alone am responsible to decide whether to participate in Classes and participation is at my own risk. Yoga is known to augment good health, but are absolutely not a substitute for medical care by a professional. HF encourages students to use good judgment. If concerned about discomfort or potential injury, I certify that I will act reasonably and immediately discontinue Classes and seek the help of professionals, including but not limited to the yoga teacher. If I am pregnant, post-natal or pre/post-surgical, my signature herein verifies that I have a physician's approval to participate. I assume full responsibility for any and all damages that may occur through participation in Classes or any activity that I elect to participate in at HF.

I am solely responsible for my personal property, including my vehicle. I agree that The HF is not responsible for any damage to or theft of my personal items.

If I am a parent or legal guardian and my child or ward is present, I take full responsibility for their actions and interactions. I will indemnify and hold harmless, HF. In addition to HF, I also indemnify and hold harmless, their affiliates, managers, members, agents, staff, volunteers, heirs, representatives, predecessors, successors and assigns. I certify that I have read these terms carefully and I am signing this agreement freely and voluntarily. The laws of The State of California shall govern this agreement. My signature below serves as a complete and unconditional release of liability to the maximum extent allowable by The State of California. By signing this release, I am waiving certain legal rights I or my heirs, next of kin, executors, administrators and assigns may have against HF. If any provisions of this agreement shall be deemed unlawful, void or for any reason unenforceable, then that provision shall be deemed severable from this agreement and shall not affect the validity and enforceability of any remaining provisions herein.
